Fremantle Community Care
Fremantle Community Care promotes and provides support to older adults and people with a disability, to maximise wellbeing, support and independence and access to the community.
Our service continues to successfully meet the seven national standards of the community west quality assurance program for delivery of community services.
The Fremantle Community Care service is funded by a number of state government agencies, and delivers the following services to the Fremantle Community:
Dial-a-ride
A Housing and Community Care (HACC) funded transport service. Transport is provided for the elderly, frail elderly, and people with intellectual disabilities. Buses travel to the heart of Fremantle, Phoenix, Gateway or Kardinya shopping centres. This service also provides weekly social outings for eligible clients. This is a fee for service program with fees set by HACC.
Freo Connections
A Housing and Community Care (HACC) funded social support program for people with intellectual disabilities, designed to engage participants in their community. This is a fee for services program with fees set by HACC.
Freo Club
A Disabilty Services Commission funded program held at the Meeting Place and other venues on Wednesday evenings. It is a chance for people to mix in a recreational setting. The group participates in planning activities for the month. A small fee is charged for attendance. Additional costs for activities such as bowling, movies, and dinner at a restaurant are paid on the day by participants. (Fee for service program).
Podiatry
Appointments for the podiatry service are available for the elderly and people with intellectual disabilities. It is a requirement to have a pensioner card or healthcare card. This is a fee for service program.
