The City of Fremantle has joined a growing number of metropolitan local governments transitioning out of providing Home and Community Care (HACC) services to elderly residents.
The decision by council comes after a major shakeup in the way home care services are funded to a centralised Commonwealth government system where clients receive funding directly to spend on the services they choose.
Fremantle Community Care will not receive any new referrals for HACC services from 1 January 2018, and will transition current clients to a new home care provider before 30 June 2018.
Existing clients will be able to access the same services through the new provider. They won’t have to do anything - it should be a seamless transition.
The City will also be sitting down face-to-face with current clients to explain what is happening and answer any questions
Fremantle Community Care is a service of the City of Fremantle offering a range of services to the community.
It also promotes and provides support to older adults and people with a disability to maximise wellbeing, independence and access to the community.

Dial-A-Ride is a transport service that operates door to door transport. Our bus picks you up from your home and travels to the heart of Fremantle, Phoenix, Gateways and Southlands Shopping Centre’s various days of the week.
This service also provides weekly social outings on Mondays. We visit places such as Kings Park, Araluen Botanical Gardens, Hillarys Boat Harbour and various Senior Citizens Centres for lunch. Both Dial-a-Ride and Social Outing services are funded by HACC (Home and Community Care) and available to people residing within the boundaries of the City of Fremantle. Fees and eligibility are set by HACC.
