Mental Health Outreach Project

The City of Fremantle hosts a Mental Health Outreach team working across the Fremantle CBD on Monday and Wednesday. A Mental Health Nurse Practitioner on Monday and Mental Health Nurse on Wednesday, work with a case workers to provide mental health support and a pathway to housing.

The team run a drop-in clinic in the Walyalup Civic Centre each Monday, 9:30am to 1pm for people experiencing or at risk of homelessness and mental health conditions.

Homeless Healthcare run the Mental Health outreach service, as well as a medical outreach service on a Tuesday. This service comprises a Registered Nurse and Case Worker, operating within the CBD and surrounds since 2019.

The Mental Health program is co-funded by the Department of Communities Local Government Partnership Fund for Homelessness and the City. The grant program aims to enable place-based responses to homelessness, create opportunities for collaborations between Local Governments and the community sector, and encourage innovative initiatives that support implementation of the State Strategy.

This service has operated within Fremantle for 12 months, and during this time have had over 300 engagements with more than one hundred individuals. As part of the program, the team liaise with local services including Police, the Library Connect Worker, City of Fremantle Community Safety team, St Pats Day Centre nurses, clinic and outreach team to ensure the best outcomes for people experiencing homelessness and mental health conditions.
